OOlite 1.77.1
Every time a start up Oolite I get the previous message in a separate window (four or five times in rapid succession, the the game crashes. It also happen on every occasion that I hold down the shift key during start up.
This can happen two or three times in a row - and then OOlite starts normally.
It also happen, at times, when I'm loading a commander (either for the first time, after a successful start up, or when I'm reloading a game.
Since I expect someone is going to say post the latest.log, I'm going to do so on the next post.
I suspect I have a conflicting or out-of-date OXP but I've checked as best I can can't work it out.

You know, I think you might be right. I've been looking over the log on here, which for some obscure reason is more readable on the browser than notepad, and I've started to spot errors that I never saw before. What are your system specs and, more specifically, RAM, video RAM and current free hard disk space? Forgetting for a second the errors reported - which should be fixed before we can reach a conclusion about the real reason for failure, the log you posted shows that it fails to allocate memory. Also the Visual C++ Runtime error message you are getting is normally associated with out-of-memory situations. This most likely means that either you have a system which is low on RAM, or you are using extremely memory demanding OXPs. You have quite a few (i.e. lots and lots) of them installed anyway, so reaching physical memory limits may be a real possibility here. This may also explain why sometimes it crashes and why some it doesn't: depending on what the populator decides to put in the start up system, sometimes memory may be sufficient, sometimes it may be exceeded.
You could try using the 64-bit version as suggested before. This would allow the game to see more memory than the 32-bit version does and then hopefully it should work without issues. Otherwise, I believe that removing some OXPs may be the way forward. And, make sure that all these errors reported in the log get corrected. Sometimes they are due to older OXP versions being in use, or maybe some dependency OXP missing, or simply buggy or incompletely packaged OXPs.

Out of memory error.
This one is almost certainly the culprit...: it requires increasingly more memory the more Ship OXPs you install, especially if your Ship OXPs have large textures.
Good news: there is a fix available in Ship Storage Helper 0.21
Bad news: the fix relies on functionality not available in any released Oolite version, so the only way to use it is to install the unstable nightly build.
You can modify the 0.20 version to use less memory, though: open theChange this to

You have quite a few (i.e. lots and lots) of them installed anyway, so reaching physical memory limits may be a real possibility here.
