Would the Prophets in Deep Space 9 count? They live in a wormhole, not sure if they are an intelligent wormhole (which is sort of a star) or some sort of parasite...

Gregory Benford's Eater features a sentient black hole - although IIRC it was a primordial black hole and therefore was not formed from the collapse of a stellar mass, so it probably doesn't count.

It doesn't. The initial star should've presented sentience, too, at least.
