Unfortunately no - as I said, I'm looking for the earliest appearance in the chronology of the stories, not in the order the films etc. were made, e.g. if the latest film had show Stan Lee in the Stone Age that might be the first appearance for the purposes of this question.

Really? Nobody? OK, let's just go for the first appearance - The second (in the chronology of the Marvel universe) is in the TV series Agent Carter, set in 1946 - Howard Stark has a clandestine meeting with someone on a shoe shine stand, Stan Lee sits down next to him and asks to borrow the sports section of his paper. The first appearance is a few years earlier.
