The consensus seems to be that this is a good idea, and as nobody has taken it further I've decide to go it alone.

I was waiting for a go-ahead - I have an Oolite logo in 3D with a nameplate section all ready to be textured...

I only went ahead as the project seemed to be both getting bogged down with 'good ideas' and seemed to be stalled - I did not want to tread on anybody's toes!
The TOGY Memorials OXP is, of course, new and is potentially modifiable/updateable so I am quite happy to swap the memorial for an improved version, and what you describe sounds very much like my original idea. If you want to send it over for a bit of texturing......

As requested, this link contains the model, OBJ version, base PNG texture, a WIP SVG file for use in Inkscape if you swing that way and a screenshot preview of the SVG's texture file in use.
Feel free to work magic on it. The texture colouring is VERY basic so feel free to gussy it up.

I had thought about dates myself, but I can think of two reasons not to. Firstly we would need to know the birth date of those featured which may not be possible, and of course Oolite time is different to 'our' time.

I don't think it really matters about the dates not being congruent with Oolite time. It's an out-of-universe memorial anyway.
