Schrödinger's Docking Sequence
I just executed a sub-standard docking (45 degree approach, with fuel injectors) and was greeted by the "Game Over" message, layered on top of the technicolour entrance animation.
Due to an earlier misunderstanding with a Q-mine, I'm wanted dead or alive. But I currently appear to be both dead and alive.

It's definitely repeatable. Just slam into the side walls of the docking tunnel on afterburners and you'll either:
a) dock
b) die
c) dock & die
d) pass right through the station
I've done each several times. When you 'dock & die' the space key doesn't do anything. It seems to be a spurious message that stays on screen for a while. You can actually relaunch and fly around with "Game Over" written across the screen. Here's a screenshot of the actual docking. Nice shields.
Also fun is launching, and holding down the fuel injector key. Nine times out of ten you smash your ship against the nav beacon. The tenth time you fly through the beacon.

It is generally inadvisable to eject over the target you just bombed.
