Whew, you actually brought an extra!
Took me a while to go through most of the references, the only weak link I would argue of is the Fast Folk (from the comments I could gather at least; besides the Culture series, I didn't read any of the titles myself, to be honest). Certainly a Culture Mind (wrapped in one of their ships) is one of the answers I was looking for.

OK, another list of five: five SF books, TV shows, or films where the main driver of the plot is that the character(s) is/are lost, and are striving to find their way home (or, possibly, to reach some other destination).

Well there is the whole of Star Trek 3 - the one with a woman in Kirk's chair, not the Earl-Grey-hot guy. Sorry - I never got into "Star Trek The Last 7 generations".
Isn't it the one with a medical hologram providing comic relief?

Unless you consider Kirk's Star Trek as being Start Trek 0 (or maybe just Star Trek), the one you're talking about should be Star Trek 4.
