I note that the name of the phantom is variably different from its genuine counterpart. Sometimes it looks more like a variable name, camelcaps with underscores. When docked, check the F3 screen and page down to near the end of the list. You should have the option to buy a Maintenance overhaul.

I stumbled on this viewtopic.php?p=296041#p296041, which on deeper inspection appears to be a telescope issue.
TLDR: Visual target of ASC entities (nav beacons and buoys/fuel stations) also appear at near zero distance on the ASC and sometimes scanner too.

I'm seeing some entities listed on this MFD at zero distance soon after jumping into a system.
